Allevant- Swing Bed Program

Roundup Memorial Healthcare has partnered with Allevant Solutions, a joint venture of Mayo Clinic and Select Medical, to develop their Transitional Care program.  Modeled on the Mayo Clinic’s successful efforts in Wisconsin and Minnesota and created by Mayo Clinic pulmonologist and Allevant Medical Director, Mark Lindsay, M.D., the program is designed to provide access to high quality post-acute services to rural America, the most persistently underserved region in modern healthcare.

Extensive Care Team

A multidisciplinary team will work together to provide you quality care


An onsite Physical Therapist provides rehabilitation services to patients with ease. 

IV Therapy

Receive IV Therapy in house under the supervision of a qualified Registered Nurse

Feel right at home

Patients who stay as Swing Bed patients are encouraged to have family visitation, dress every day, and participate in daily activities.  Patients are encouraged to complete daily tasks as if one was at home.

Do I qualify?

Patients who have been hospitalized for at least 3 midnights, but require extra care such as rehabilitation services, would benefit from the Swing Bed Program.

Quality Care

Roundup Memorial Healthcare has a Physician,  Physician Assistants, Registered Nurses, a Physical Therapist, and Certified Nurses Assistants in addition to high end Radiology and Laboratory services on staff. This will ensure quality care and a seamless connection between your healthcare providers.
